ATTENTION: COMMUNICATION WITH PROVIDERS: We do not communicate with patients via e-mail. For urgent matters, call 911. For less urgent matters that are best handled by the telephone or a face-to-face encounter with your clinician, call the office to schedule an appointment. Calls longer than 5 minutes will be billed as an appointment according to our non insurance hourly fee schedule. Please do note that insurance companies typically do not pay for phone sessions. So please make your call productive. At this point we have not anticipated having to bill for the 5 minute calls. However, excessive use of this outside of session will be considered and will be billed as an appointment according to our non insurance hourly fee schedule.

Psychiatry Hours Mon 1 pm – 6 pm, Tues 10 am – 7 pm, Wed 10 am – 6 pm, Thurs 10 am – 7 pm
Psychology Therapy Hours Monday – Thursday 9 am – 8 pm.
Saturday Hours: Psychiatry 10 – 5 pm held 2-3 Saturdays per month Psychology 9 am – 8 pm held 3-4 Saturdays per month
Group Therapy: Wednesday & Thursdays 6-7 pm Children, 7-8 pm Adolescents

24 Hour Cancellations ONLY Individual child/adolescent/adult appointments.
48 Hour Cancellations ONLY New Patients & Multiple Family Members
48 hour or Greater Cancellations ONLY MONDAY/SATURDAY situations
1. Individual Appointments: must cancel 24 hours before your appointment, to avoid a late cancellation or no show fee except on MONDAY/SATURDAY cancels. Families (multiple family members in therapy or psychiatry with separate appointments on the same day or consecutive days) & must cancel 48 hours in advance of the appointmentexcept on MONDAY/SATURDAY cancels. FAQ EXAMPLES & DIRECTIONS ON MONDAY/SATURDAY EXCEPTIONS: (These are only examples. Please ask reception for further clarification if you have any questions. Please clarify at least 1 week before your appointment about your cancellation situation is. You can always call us back to clarify if your appointment needs to be cancelled on a Thursday. QUESTION: How do I find out if FBH is Closed any particular weekend? ANSWER: Look at your calendar 1 week or more before the appointment and see if you fall in the MONDAY/SATURDAY cancel time frame. Then call the clinic and ask them if we are open that particular weekend. Our voice mail system has a time/date stamp letting us know when you called. So please leave a message at 8 am if no one is available to take your call. 2. QUESTION: What do I do if I need to cancel on Monday & FBH is Closed this weekend? ANSWER: If your appointment is on a Monday and it falls on a weekend the clinic is closed then you must cancel by Thursday 8 AM to give us enough time to try to fill your appointment time. If you call on a Sunday or a holiday or non clinic weekend then we will not have enough time to find someone to fill that slot and you are responsible for your appointment. If you call at 8 AM on Saturday to cancel for a Monday appointment then you are OKAY as long as we have clinic on that Saturday. QUESTION: What do I do if I can not make it to my appointment on Saturday? ANSWER: If your appointment is on a Saturday then you must cancel by Thursday 8 AM. Electronic Refills: ASK your PHARMACIST to SEND an ELECTRONIC REFILL REQUEST. Please ask for refills 1 week before your medications run out. Call your pharmacy for Electronic Refills. First time medications need to be sent by FBH first. Stimulant Prescriptions: We know how busy your lives are. Please call for your refill 7 days before your prescription runs out. Calling early allows you time to come get your prescription and give it to your pharmacy. LOST PRESCRIPTION FEE: Please do not lose your refill prescription(s) because there is lost prescription fee. Please refer to Illinois Public Act 096-0166 Effective Date 1/1/2010: …A prescription for a Schedule II controlled substance shall not be filled more than 90 days (used to be 7 days) after the date of issuance….
